Home-Based Care in Your Community
Home health care and personal care are vital in-home services that help people of all ages throughout Colorado.
From infants to the elderly, people receive home health and personal care services for a wide range of situations and needs, such as following an accident or a medical procedure, or because of an impairment, disability or chronic illness.
Home health care services are provided by a variety of licensed health care professionals in the home and can include skilled nursing care or other skilled care services such as physical therapy, occupational therapy, speech-language therapy or services from a medical social worker.
Personal care allows a person with special health needs to continue to live in their home and can include assistance for seniors, adults or children with chronic illnesses, patients who are recovering from surgery or those living with an impairment or disability.
Services can include:
- Bathing, dressing and toileting
- Assistance with mobility, including standing, walking and sitting
- Help with meal preparation and feeding
- Medication reminders
- Help with grocery shopping and personal errands
These services allow people to receive care without the need to be in a hospital or institutional setting.
